That pressure test is the critical part—it catches the hairline cracks that cause hydrolocked cylinders later. For a daily-driven truck, this is what I recommend. The extra cost buys certainty, and the labor is the expensive part anyway.

New Aftermarket Heads—A Mixed Bag

New heads go for about $300–500. Some are excellent. A few are junk. I've seen a brand-new head with a porous casting that wept coolant through a pinhole only after the engine reached operating temperature. The warranty covered the part, but the customer still paid $400 in labor to swap it a second time. If you go this route, inspect the casting carefully and pressure-test it before installation.

Used Heads—The Gamble

On a modern transverse-mounted car, the alternator might be tucked low behind the splash shield, under hoses, a bracket, and the intake duct. That's a three-hour job. The part is the same—the effort is completely different. So the answer to "is it hard?" depends entirely on what you drive.

The Diagnosis People Skip

What I see most often is someone replacing an alternator without testing the battery first.
